Make most out of chronic care management resources!!!

Chronic diseases, such as diabetes, heart disease, and hypertension, require ongoing care and management. To provide comprehensive support for individuals dealing with chronic conditions, Chronic Care Management (CCM) resources have become essential. These resources offer a structured approach to managing chronic illnesses, improving patients' quality of life, and reducing healthcare costs. Chronic care management resources typically include a combination of healthcare professionals, technology, and patient education materials. Healthcare providers, including nurses, physicians, and care coordinators, collaborate to develop personalized care plans for patients. These plans encompass regular check-ins, medication management, lifestyle guidance, and remote monitoring. Medication treatment monitoring is a crucial component of CCM resources. This involves tracking and managing medication regimens to ensure that patients adhere to their prescribed treatments. Medication adherenc

Exploring Chronic Care Management Resources and Digital Medication Monitoring

Chronic diseases pose a significant burden on healthcare systems worldwide, requiring comprehensive and continuous management. In recent years, healthcare providers have embraced innovative solutions such as resources for chronic care management and digital medication monitoring to improve patient outcomes and enhance the quality of care. Chronic Care Management Resources: Chronic care management (CCM) resources encompass a range of tools, technologies, and services designed to support patients in managing their chronic conditions more effectively. These resources facilitate communication, education, and coordination between patients, caregivers, and healthcare providers. The goal is to optimize care delivery, improve patient engagement, and reduce hospitalizations. One key component of CCM resources is the implementation of electronic health records (EHRs) and patient portals. EHRs consolidate patient information, allowing healthcare providers to access comprehensive medical h

Bakersfield firm launches software for remotely monitoring COVID-19 cases

A Bakersfield-based medical software company has responded quickly to the coronavirus pandemic with its launch Tuesday of a cloud-based computer platform designed to help physicians remotely monitor COVID-19 patients. Sargas Pharmaceutical Adherence and Compliance International's system uses Bluetooth-connected wireless medical devices — or, if that's not available, manual data entry — to record patients' temperature, oxygen saturation, blood pressure, pulse and weight. That information is then shared with caregivers over a secure internet connection. CEO Gitesh "Git" Patel, who has worked closely with Bakersfield's Comprehensive Blood & Cancer Center for many years, said his roughly 30-employee company has worked in the field of remote patient monitoring for several years. It now provides non-COVID-19 illness management services to hundreds of doctors and some 15,000 patients around the country. When the coronavirus pandemic arose, the company looke

SPAC International: Creating Seamless Connection Between Healthcare Technology And Patients With Human Touch

In the current healthcare scenario, value-based care has become increasingly dependent on technology since modern digitization has significantly augmented it. Emerging advancements like AI and APIs assist in meaningfully integrating data to deliver a high quality of care. Likewise, it helps in reduction of hospitalizations, ER visits, and nursing home visits, which ultimately decrease healthcare expenses. However, according to Git Patel, the CEO of SPAC International, the present technologies still lack some intelligent features which only a human can do. One can say, technology is not fully developed yet to act as a human being. To further elaborate, he gives an example of a patient using wearable to track daily activities. In this case, if the patient forgets to wear the device, then his complete efforts go waste. This applies to every patient utilizing technology. Therefore, to avoid such complications and disengagement of patient and technology, Git’s company brings human assistanc
